What if I want a copy of my STD test results?
You can request a free copy of your test results from our Medical Records Unit by completing a Release of Information form, available from the receptionist. Copies of test results are provided Monday through Friday, from 9 a.m. to 11 a.m. and 2 p.m. to 4 p.m. A photo ID will be required to obtain copies of your test results. To maintain patient confidentiality, copies of test results will only be given if the name and date of birth that you gave during the initial registration process matches exactly with the name and date of birth on your photo ID.
