Ending Homelessness
The Maricopa County Health Care for the Homeless Program Opens a New Window. uses an integrated model of health care delivery servicing all of Maricopa County. Services include outreach, primary care, case management, housing, behavioral health services, substance abuse counseling, and referrals for dental, vision, and entitlement programs.
No one who has served our country should ever go without a safe, stable place to call home. All too frequently, veterans are unable to avoid entering the county's homeless population. The Maricopa County Public Health and Human Services Departments work in conjunction with many different partners on Project H3 Vets, which is a community initiative coordinated by the Arizona Coalition to End Homelessness. The initiative, which seeks to end chronic homelessness among veterans, is aligned nationally with the 100,000 Homes Campaign and the VA National Homeless Veterans Outreach Campaign.
Home Improvement
The Maricopa County Human Services Department is pleased to offer veterans assistance with modifying their home to provide greater accessibility through the Service Disabled Veterans Program (HOSDV). Eligible improvements are disability modifications to owner-occupied homes of veterans that have a service-related disability. The HOSDV loan program funds home modifications that enhance accessibility which include, but are not limited to: widening doorways or hallways; lowering countertops; adding exterior ramps, and installing grab bars or roll-in showers in bathrooms.
